Appointment Policies:

New clients:

  • We ask that you please send your pet’s records in advance to help us prepare for your visit. You may have your records mailed to us at [email protected]. This will ensure you have the most efficient appointment possible – thank you!

Deposits:

  • To save your appointment time with the doctor, we will collect a deposit equal to the exam fee. The deposit is forfeited if no-showed or not canceled at least 24 hours prior to the appointment.

Please note: If your appointment is on a Monday, you must cancel on Friday.

Payment Policy: Fees are due at the time services are rendered. There are no payment plans, and we regret that we can no longer accept checks as payment. We recommend Trupanion Pet Insurance to help cover veterinary costs if needed. Care Credit and Scratch Pay are additional payment option. 43rd Avenue Animal Hospital maintains strict sanitary conditions to prevent the spread of infectious disease. All hospitalized/drop off patients must be properly immunized and free of internal and external parasites. Pets not current on immunizations and/or with internal/external parasites will be vaccinated and/or treated at the customary fees. I agree that 43rd Avenue Animal Hospital may use photographs of me or my pets with or without my name and for any lawful purpose, including such purposes as publicity, illustration, advertising and online content.
